9) Can the JuanLife member avail of the extended benefits even if they didn’t purchase their eyewear from Vision Express?
No. Eyewear must be purchased from Vision Express to avail of the extended benefits.

10) Can the JuanLife member avail of the extended benefits even if they didn’t purchase their Vision Express eyewear within the contract validity period?
No. Eyewear must be purchased from Vision Express within the contract validity period in order for them to avail of the extended benefits.

11) How will the JuanLife member know if the eyewear they have chosen is applicable for the discounts?
They may approach and ask the staff in the store.

12) Are there exclusions on the 1-year warranty?
The warranty does not cover change of mind, accidents, misuse, and/or wear and tear.

Vision Express will verify the name of the beneficiary on their list. The beneficiary must present a valid ID as proof of identity.

14) What if the JuanLife member doesn’t have specified beneficiaries in their JuanLife account?

15) Can the JuanLife member book an appointment prior to the store visit?

16) What are the modes of payment?

17) If the JuanLife member’s policy is about to expire, can they still avail of the discount?
They can avail of the discount as long as it is within the validity period.

18) If the JuanLife member’s account has already expired and they purchase a new policy, can they avail of the discount?
Yes, they can avail of the discount under the new policy as long as it’s activated within the validity period. They can present their new COI to the Vision Express staff.

19) Can the benefits be transferred to family members and friends?
No. Only the JuanLife member with the COI and the listed beneficiaries on their account can avail of the free Eye and Visual Health Screening, discounts, and extended benefits.

ANNEX A

Eye and Visual Health Screening

• Auto-Refractometry – usually the first diagnostic examination. The initial method to determine refraction (diopter). The examined person presses the chin and the forehead on the holder of the special instrument called autorefractometer. The principle is to notice the image that is projected on to the retina. The instrument automatically measures the diopter, and the process of measuring lasts only a few seconds. The doctor then determines what diopter is needed to achieve the clearest picture on the retina.
• Testing Visual Acuity – measured by the ability to discern letters or numbers on an eye chart at 20 feet distance. Visual Acuity means clarity or sharpness of vision and that the objects you see are crisply outlined and not blurry.
• Ophthalmoscopy – Direct Ophthalmoscopy – the examined person is seated in a darkened room where the examiner performs the exam by shining a beam of light through the pupil using an ophthalmoscope. This allows the examiner to view the back of the eyeball. Indirect Ophthalmoscopy – the examined person will either lie or sit in a semi reclined position. The examiner holds your eye open while shining a very bright light into the eye using an instrument worn on the head. The examiner views the back of the eye thru a lens held close to your eye. The person being examined will be asked to look at various directions. This exam is usually used to look for a detached retina.
• Retinoscopy – also called sciascopy. This is a technique to objectively determine the refractive error of the eye (farsighted, nearsighted, astigmatism) and the need for eyeglasses.
• Binocular Vision Test – Binocular Vision is the vision achieved by the coordinated use of both eyes so that the images arise in each eye separately are appreciated as Single Mental Impression in visual part of cerebral cortex. This test allows the doctor to detect a number of vision issues that might otherwise go unnoticed during a standard eye exam or vision screening. Visual skills evaluated in this test are: fusion, convergence, working memory, visual integration, visual perception, processing speed, depth perception (3D), vergence (eye teaming), ocular posture (resting position), ocular motility (eye movements), spatial awareness and visual attention, accommodation (eye focusing skills)
• Subjective Refraction – is a technique to determine the combination of lenses that will provide the best corrected visual acuity (BCVA). It is a clinical examination used to determine a patient’s need for refractive correction, in the form of glasses or contact lenses. Its goal is to achieve clear and comfortable binocular vision.
• Duochrome Test – each eye is tested separately to find out if eye is over corrected or under corrected by a duochrome chart. Whichever color is focused nearest to the retina will be seen as clearest.Emmetrope = equalMyope = red is clearer (add +.25DS, Over corrected of hypermetropia)Hypermetrope = green is clearer (add -.25DS, Over corrected of myopia)

  End point is obtained when the letters on the Red-Green chart appears equally dark or when a reversal occurs.
